Issue
Base image preparation fails with McAfee AntiVirus enabled. Status log errors appear when the WinPE media is created, for example:
Error: Attempting to add winpe-hta returned: 5 [Access is denied]
Solution
The issue caused by an issues between DISM.exe and the McAfee software. Review McAfee Technical Article KB76867
